PHYSICIAN AND MEDICAL OFFICE REFERRALS:

 

 

Thank you for referring your patients to us. We are committed to providing them with the best care possible. We are also committed to helping you better serve your patients in the mental health and substance abuse areas. Several tools are available to help do this:

 

The Physician Referral Form provides an online version of our standard referral form that you can download and print when referring patients to us.

 

 Referrals are to be made by completing the attached form  and sending it to  admin@movingaheadnc.org  or by faxing it to (910)253-5887.

 

 

Please note that  we are not currently taking any referrals for Psychological Testing at this time.

To save time, patients can access our intake paperwork directly via this website, or we will contact them to complete registration once we receive your referral.  Please note that referrals are NOT needed to access services, but any information you can share prior to the patients first appointment will help us find the right clinician for your patient.
